The Christ Hospital Health Network Clinical Navigator in CINCINNATI, Ohio
Shift: Full-Time, Days
This position will be in our Oncology/Hematology Office located at 4460 Red Bank Rd.
Job Overview: The Clinical NavigatorServes as the day-to-day lead within the Hematology-Oncology Center and supervises the development, implementation, and organization of the overall program. Coordinates the efforts between physicians, Oncology Service line, staff and community hospitals. Responsible for patient care coordination, report generation and review, process improvement, patient satisfaction, and administrative activities within the Center.
The Clinical Navigator may perform the following duties:
Facilitate communication between the Center medical director, physicians, key staff and patients.
Develop standardized care plan with the physicians including: referrals, bone marrow transplants, drug protocols and clinical trials
Ensure appropriate bone marrow preparation for every patient.
Develop and deliver patient education and consultation.
Direct patient participation in phase studies, drug protocols or trials.
Round with physicians daily to gather required data for in patient care including daily labs, test results, etc.
Liaison between physician and hospital care team and/or ambulatory care centers to ensure seamless delivery of care.
Oversee the development and on-going editing of specific forms and patient education material
Develop a community education and seminar program inclusive of schedules, presenters, content and materials.
Create and oversee an ongoing quality improvement plan.
Identify patient amenities and secure necessary resources to administer the services.
Education:High school diploma or GED required. Clinical Hematology Oncology background – R.N., Oncology Certification Required. Master’s degree preferred.
Experience:Minimum of 5 years of experience preferred.

Proven working relationship with surgeons.
Experience with patient / staff education.
Proven leadership experience having managed resources either directly or within the implementation of a new imitative.

License & Certification: Current Registered Nurse license or interim permit in the State of Ohio.
